We sometimes ask patients to provide us with some home blood pressure readings. We have monitors to loan out to patients; alternatively many patients choose to purchase their own. We have produced a standard form to use with either our machines or with patients own. Please download and complete the form from the link below and then drop it into reception, and the results will be reviewed by your GP.
