TRAVEL ADVICE

We offer appointments - either by phone or face-to-face - with our practice nurses to discuss your health needs prior to travel overseas. It is very helpful to have some information from you prior to this discussion and for this reason we have produced a pre-travel questionnaire. Please start this process eight or more weeks before you travel; any later than four weeks can cause real difficulties.

Please note there are charges for some travel vaccinations, as they are not all provided by the NHS. Also tablets to prevent malaria are only available on a private prescription, for which there is a charge. The nurse will tell you whether you need these or not.

Please print off the and complete the Pre-Travel Questionnaire and return it to the surgery prior to your appointment with the practice nurse.  If it is more convenient to email it please send it to general.office@gp-A82053.nhs.uk
